Post-acute care focuses on assisting patients after a hospital discharge, working with physicians and nurse practitioners to provide seamless care in transitioning patients from the hospital to what best fits an individual's circumstances, whether that is inpatient rehabilitation, a nursing facility, or something else.
Long-term care focuses on placing patients in the proper facility and providing all necessary services for their condition and ability, ensuring a smooth transition from home or hospital to nursing facility, then possibly to assisted living or other location to receive care. This type of care is received by all patients who experience an illness or injury that affects their ability to live independently, but is typically associated with older patients. It often overlaps with, though is not the same as, the field of geriatrics.
Geriatric medicine focuses on promoting health and preventing and treating disease and disability in later life. Our providers specialize in the prevention and management of the unique health concerns associated with aging; they provide treatment, manage multiple disease symptoms, and develop care plans that address the specific needs of adults age 65 and older.
